La imagen social que posee sobre los Profesionales de Enfermería-en cuanto a su labor-, las personas del Barrio Santa Rita que utilizan el servicio del CAPS N° 16 de la ciudad de Posadas, Misiones

Abstract

La Disciplina Enfermera tiene como objeto de estudio el cuidado humano, a partir del cual plantea un cuerpo de conocimientos propios representados por teorías y modelos que son la base de su quehacer. Su devenir histórico se produjo en consonancia a las corrientes paradigmáticas que se han sucedido a través de la historia, favoreciendo su consolidación como disciplina autónoma, cuya actuación independiente o en colaboración, favorece la aplicación del Proceso de Atención de Enfermería, como metodología científica que contribuye a brindar cuidados de calidad 1. La presente investigación indaga el tipo de imagen social que poseen las personas del Barrio Santa Rita que utilizan el servicio del CAPS N° 16 de la ciudad de Posadas, Misiones, sobre los Profesionales de Enfermería, en cuanto a su labor, en el año 2018. A partir de ello se propuso describir aquellos aspectos de la imagen social que denotan los niveles de formación y las competencias que, según la comunidad del Barrio Santa Rita, poseen los profesionales de enfermería. El supuesto que se sostiene en la presente investigación es que la imagen social que tienen quienes acuden al CAPS, es parcializada, en cuanto al perfil y rol profesional, donde prima un desconocimiento de las competencias y el nivel de formación que poseen los profesionales de enfermería. Para corroborar este supuesto, se realizó un estudio cuali-cuantitativo de carácter descriptivo. La población estuvo compuesta por todos los habitantes del barrio Santa Rita que asistieron al Centro de Atención Primaria de la Salud (CAPS) No 16, y mediante un muestreo no probabilístico por conveniencia, se seleccionaron 30 unidades de análisis. La recolección de datos se llevó a cabo mediante una entrevista, (grabada en soporte audiovisual) con una guía de preguntas abiertas lo que permitió analizar cada relato y a partir de dicha información se establecieron las categorías, subcategorías y sus dimensiones; teniendo en cuenta las regularidades o patrones de las respuestas de cada unidad de análisis; dicha entrevista fue aplicada por el investigador a cada unidad de análisis que aceptó participar del estudio en forma voluntaria y anónima, previo consentimiento informado firmado. El análisis de datos se efectuó por segmentación, codificación, categorización y búsqueda de regularidades, analizando la información obtenida a la luz del marco teórico construido. La presentación de los datos se plasmó mediante fragmentos de relatos, elaborados a partir de los resultados de las entrevistas aplicadas. Los resultados de esta investigación contribuirán al desarrollo de la imagen social del enfermero como persona cualificada, fortaleciendo la autonomía y la identidad profesional mediante la socialización del quehacer propio de la disciplina. De tales resultados emergieron las siguientes categorías. Como categoría general, la Imagen Social del Profesional de Enfermería, la cual se ha segmentado en tres subcategorías, 1. Información, 2. Actitud y 3. Campos de las representaciones sociales. A su vez, cada una de estas subcategorías se ha vuelto a dimensionar en categorías más específicas luego de ser halladas las regularidades en las diferentes respuestas aportadas por las unidades de análisis. Partiendo de la base de qué consideran es la enfermería (concepto); se concluyó que la comunidad del Barrio Santa Rita aún mantiene en sus concepciones del profesional de enfermería una fuerte influencia de sus orígenes en cada una de sus etapas de los cuidados: la Etapa Doméstica, la Etapa Religiosa o Vocacional, la Etapa Técnica (la influencia de la medicina), así como también la Etapa Profesional. Esto se debe a que destacan dentro de las definiciones la ayuda, atención, curación y, en menor medida, el cuidado. En cuanto a las características necesarias que debe tener este profesional, se mencionan la amabilidad, el servicio, la vocación y la cordialidad, entre otros. En cuanto al nivel de formación que deben tener dichos profesionales, todos coinciden en que deben contar con algún tipo de capacitación, conocimientos y habilidades que permitan dar cuenta de su que hacer profesional; sin embargo, el nivel de titulación aún no queda del todo claro. En relación con las competencias y funciones del profesional de enfermería, las más visibilizadas y reconocidas por la sociedad están vinculadas a las tareas (técnico, procedimental) las relacionadas al ámbito asistencial, y si bien las competencias de docencia, administración e investigación fueron citadas como necesarias; no se identifica claramente a qué tareas corresponden cada una de estas competencias. Además, y en menor medida, se desconocen o se niegan esas competencias como necesarias en estos profesionales. No obstante, en cuanto a las funciones dependientes o en colaboración con los demás profesionales de la salud, sigue inminente la idea de que el enfermero cumple órdenes médicas y debe realizar tareas que son impuestas por el facultativo; esto refleja que los profesionales de enfermería aún tienen mucho trabajo por hacer en cuanto a la divulgación de las competencias que avalan su accionar.

The subject of study of the discipline Nurse is the human care, based on which it contemplates a body of characteristic knowledge represented by theories and models that are the base of its work. Its historic development happened together with the paradigmatic currents that have developed through history, favoring its consolidation as an autonomous discipline whose performance, be it independent or in colaboration, supports the application of the Process of Nursing Care, as a scientific methodology that contributes to providing quality care 1. The current research looked into the type of social image that the people from the Barrio Santa Rita that use the service of the CAPS N°16 of the city of Posadas, Misiones, have about the Nursing Professionals -with regard to their work- in the year 2018. Based on that, the proposal was to describe those aspects of the social image that indicate the levels of training and the competences owned by the nursing professionals, according to the community of Barrio Santa Rita. The case upheld in the current research is that the social image held by those who attend the CAPS is partialized (regarding the professional profile and role), where a lack of awareness prevails as to the competences and level of training possessed by the nursing staff. For that purpose, a qualitative and quantitave descriptive study was carried out. The population was composed of all the residents of the Barrio Santa Rita that attended the CAPS N°16 (CAPS stands for Centro de Atención Primaria de la Salud, Spanish for Center of Primary Health Care); from which 30 units of analysis were selected through a convenience non-probabilistic sample. The data collection was performed through an interview (recorded on audiovisual format) with open-guided questions that allowed for the analysis of each story and, based on the resulting information, the categories, subcategories and their dimensions were established, taking into account the regularities or patterns of each unit of analysis. Said interview was applied by the interviewer to each unit of analysis that voluntarily and anonimously accepted to participate of the study, prior informed and signed consent. The data analysis was carried out by segmentation, codification, categorization and search of consistencies, analysing the gathered information in the light of the built theoretical framework. The data was presented through fragments of narrations, based on the results of the applied interviews. The outcomes of this research will contribute to the development of the social image of the Nurse as a skilled person, strenghtening the autonomy and the professional identity through the socialization of the characteristic work of the discipline, from which the following categories emerged. As a general category: the Social Image of the Nursing Professional, which has been segmented into three subcategories: 1. Information; 2. Attitude; and 3. Fields of the Social Representations. In turn, each of these has been rescaled into more specific categories after the found consistencies in the different responses provided by the units of analysis. On the basis of what they think Nursing is (concept), it was concluded that the community of the Barrio Santa Rita still maintains in their conception of the nursing professional a strong influence of the profession’s origin in each of the stages of caring: the Domestic Stage, the Religious or Vocational Stage, the Technical Stage (the influence of medicine) and the Professional Stage. This is based on the fact that the definitions include a focus on help, assistance and recovery and to a lesser extent the caring portion of the discipline. Regarding the necessary characteristics that this professional should have, kindness, service, vocation and warmth, among others, arose during the interviews. As to the level of training that said professionals should possess, all the participants agreed on the fact that the nursing professionals need to rely on some sort of training, knowledge and abilities that allows them to be held accountable for their professional work. However, the level of degree that the Nurse should have is not yet very clear. With regard to the competences and roles of the nursing professional, the most visible and acknowledged by the society are linked to the tasks (technical/procedural), the ones related to the welfare field, and even though the tasks related to teaching, administration and research were cited as necessary, it is not completely identified which tasks each of those competences correspond to. To a lesser extent, these competences are not acknowledged as necessary in these professionals; however, regarding the dependant or collaborative roles with the rest of the health professionals, the idea is still present that the Nurse follows the orders of the physician and must perform tasks that are imposed by said physician. This reflects that there is still a lot of work to be done on the part of the nursing professionals regarding the disclosure of the competences that validate their actions.
